Job Description

Licensed Medical Social Worker (LSW/LCSW) Full-Time | Somerset & Union Counties, NJ BAYADA Home Health Care is seeking an experienced and compassionate Licensed Medical Social Worker (LSW/LCSW) to join our Union County Visits team. This full-time position provides one-on-one home health visits to adult and geriatric clients throughout Somerset and Union Counties, NJ. As a Medical Social Worker, you'll play a vital role on an interdisciplinary home health team, helping clients and their families navigate the social, emotional, and environmental challenges that impact their health and well-being. Your expertise will connect clients with community resources, provide advocacy, and help them safely remain in the place they call home. Must hold an active New Jersey Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) license. Home health experience is preferred, but not required. Responsibilities Conduct in-home psychosocial assessments and clinical social work evaluations. Develop individualized intervention plans based on client and family needs. Assess social, emotional, financial, and environmental factors affecting health outcomes. Connect clients and families with appropriate community resources and support services. Advocate for clients and families while helping them navigate healthcare and community systems. Collaborate with nurses, therapists, physicians, and other interdisciplinary team members to develop comprehensive plans of care. Build and maintain relationships with community agencies and resource providers. Educate clients and caregivers on available community resources to promote independence, safety, and overall well-being. Complete timely and accurate documentation using BAYADA's electronic medical record (EMR) system. Qualifications Active New Jersey Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) license. Master's degree in Social Work from a Council on Social Work Education (CSWE) accredited program. Minimum of two years of community-based social work experience. Home health experience preferred, but not required. Strong communication, interpersonal, and organizational skills. Ability to work independently and effectively manage time. Experience with electronic medical records (EMR) preferred. Reliable transportation and ability to travel throughout Somerset and Union Counties. Why BAYADA?
